HOW TO REGISTER AS A PATIENT:
If you live in our practice area and are not presently registered with a local practice, please present yourself in person and bring your medical card which will have your personal information regarding date of birth, previous address and previously registered GP, and your NHS number. Although it is better to present with this card as it speeds up your records being transferred from your previous GP, do not worry if you cannot find it. We do have a form at the practice that you will be able to complete. If there are other members of your immediate family who cannot attend at the same time, it will be necessary for them to attend at some point as upon acceptance of your registration, you will be offered a health check with the practice nurse. This is not necessary for children under 5 years.

APPOINTMENTS:
All surgeries and clinics are by appointment only and may be made by telephone or in person at reception when the surgery is open. We try to keep a few appointments free each day for emergencies. To help the staff please state if your problem is urgent and requires attention that day. MEDICAL emergencies will be seen ASAP. All urgent requests for appointments or home visits will be triaged by a practice nurse. Patients are requested to take the advice of the clinical staff, as to whether a home visit or surgery consultation is necessary. Please try to advise reception if you are unable to keep any appointment as it may be offered to another patient.

HOME VISITS:
Please remember that the Doctor can see several patients in the surgery in the same time that a single home visit takes.

If you require a home visit please telephone before 10am if possible as the Doctors plan their work early in the day. When requesting a visit you will be asked the nature of the problem to assist the Doctor to decide the urgency of each case. Please note that this service is for patients who are too ill or infirm to attend the surgery and not for patients who have no transport.

WHEN YOUR CHILD IS ILL:
The vast majority of sick children can be safely seen in the surgery. A home visit is not usually necessary. It is often quicker for the child to be seen in the surgery, and Doctor will see the sick child as soon as he is free, with a minimum of waiting. If you are unsure if your child is fit to travel please telephone the surgery for advice.

REPEAT PRESCRIPTIONS:
We do not take repeat prescriptions over the phone because of the safety aspect. Please let us have a copy of your latest prescription duly marked with the items required. You may also post the request in, but please remember to enclose a stamped addressed envelope. Patients in Brymbo may leave their requests with the Brymbo Chemist.

We also have a prescription collection arrangement with local chemists. Please let the receptionist know if you would like to collect your prescription in this way, to save attending the surgery again. Unfortunately we do not cover all chemists - only Boots and Rowlands.

Please allow 48 hours before collection.

Patients wishing to order repeat prescriptions online may do so by using the form on this website. You should be aware that submission of this form will involve transmitting personal details about yourself across the Internet. Your information is encrypted during transmission, but we cannot offer an absolute guarantee of privacy and use of this method is entirely at your own risk. However when the information reaches the practice it will be held totally secure. If this matter is of concern to you then you should use one of the alternative methods of ordering a repeat prescription. Please note that we cannot accept repeat requests via email as they may contain insufficient information to allow the issue of a repeat prescription

NON NHS SERVICES:
Dr. Gupta is happy to carry out medical examinations for such purposes as insurance, medicals etc. fitness to travel etc. The NHS does not cover these services so there will be a fee. Please enquire at reception.

FLU VACCINATIONS:
We do recommend immunisation against flu for all patients suffering from - asthma, emphysema, heart or kidney problems or diabetes. All patients over 65 years of age and anyone living in a nursing home. These are normally commenced in September. Please make an appointment.

TRAVEL VACCINATIONS:
Patients travelling abroad should check whether vaccinations are required for the country they are visiting. Advice may be obtained from and vaccinations administered by the practice nurses. Please make an appointment.

OUT OF HOURS:
IF THE SURGERY IS CLOSED AND YOU HAVE A GENUINE LIFE THREATENING EMERGENCY PLEASE CALL 999
If you require URGENT medical attention when the Surgery is closed contact North Wales GP Out of Hours Service (as endorsed by Betsi Cadwaladr University Health Board) by calling 0300 123 55 66.
Alternatively, you may telephone the Surgery on 01978 367921 and follow the emergency service telephone instructions.
North Wales GP Out of Hours (OOH) Service is open:
Overnight (6.30pm to 8am)
Over the weekend (6.30pm Friday until 8am Monday)
On Bank Holidays

ZERO TOLERANCE POLICY
The Practice operates a zero tolerance policy whereby any patients who are violent or abusive to any member of the practice team will be removed from the practice list.
