Our Aims

We want to provide you with the best possible health service.

Our relationship must be based on mutual respect. We are partners with you in health care. We all, patient, doctor and practice staff, must work together showing respect and courtesy to each other. We will then be able to provide you with the most appropriate care, meeting your own individual health and personal needs, and with your full co-operation and informed consent. We listen to and take account of your views in the running of the practice.

In this practice the doctor, nurses, administration and reception staff work as a team. We are committed to meeting your needs at the appropriate time and place. In an emergency medical treatment or advice is always available.

In addition:

We try to arrange an appointment with the doctor within two working days.

If the doctor cannot see you within a reasonable time ( for your appointment ) you will be told why and offered the choice of waiting or agreeing an alternative time.

We will ensure your privacy and confidentiality when you speak to the doctor or other team member.

Your medical records are held in the strictest confidence. Information is not passed on without your consent unless it is within the confines of the NHS, by legal framework, or is in the public interest. Certain anonymised patient data may be shared for the purposes of public health and audit, research, teaching and training. This practice is registered under the Data Protection Act. It is a practice and legal requirement that all staff maintain confidentiality of patients' records.

Health promotion and general information is available and on display on the practice premises, supporting the advice we provide to you.

Outside normal surgery hours you can contact a doctor by making usually only one phone call.

If you are unsure of the names and duties of staff members please ask and this information will be made available to you.

As part of our health promotion policy all our surgery premises are designated as NO SMOKING  and NO PETS ( except guide dogs ) areas.

Registered patients aged 16-74 who have not been seen for three years may request a consultation.

Registered patients aged over 75 years who have not been seen in the previous 12 months may request a consultation. If you are unable to attend the surgery for this consultation because of your medical condition, a home visit may be arranged.

The doctors nurses and practice manager are happy to speak to you by telephone at a mutually convenient time

We will explain clearly how and when you can obtain the results of any tests we arrange for you.

PATIENTS WITH SPECIAL NEEDS:

If you have any special needs or difficulties please discuss them with the doctor or other member of staff. We will do our best to make appropriate arrangements.

Our premises are not wholly suitable for wheelchair access although it is possible to access the main door and the GPs' consulting rooms.

However, as the Nurses consulting rooms are situated upstairs (at Wrexham) if you think you will struggle with the stairs please let the reception staff know and they will then mark your notes "to be seen down-stairs" so that when you ring in for an appointment, the computer system will flash up a warning for the staff to book you in when a down-stairs room is available.

Although the practice parking is on the road at the rear of the surgery, it is permissable to park vehicles, as long as they do not block access to Fairy Road (for safety reasons). This allowance is only for those with mobility problems.

Our branch surgeries in Brymbo and Brynteg are suitable as they are in local authority premises.

For patients with hearing difficulties, we have a portable loop system available.

Tell us when you change address or other personal details, the return of undelivered mail may result in your removal from the practice list.

REMEMBER YOU ARE RESPONSIBLE FOR YOUR OWN HEALTH.

  • If you drink alcohol, do so in moderation.
  • DO NOT smoke.
  • DO NOT misuse or abuse other drugs.
  • Take regular exercise.
  • Eat a varied healthy diet.
  • Practice safe sex.
